Fertility Health Assessment

What you need to do now:

1. Download and complete the fertility health assessment.

2. Email your completed assessment and up to 5 blood test files (PDF or image format) to:
   admin@carmenmair.com

3. Be sure your full name and test dates are visible on all documents.

4. Include in your email:

   • The cycle day the labs were taken

   • Any specific questions or concerns (e.g., “Why is my progesterone low?”)

   • Any relevant history (e.g., miscarriages, medications, recent stress)

   • Which week of the 12-week Fertility Program you are currently at

Please allow up to 5 business days to receive your personalised lab report via email.
